•The association of Wernicke encephalopathy and alcoholic liver disease in patients with alcohol use disorders is frequent.

•Diagnosis of Wernicke encephalopathy in patients with alcoholic liver disease can be harder due to clinical differences.

•Patients with alcoholic liver disease and wernicke encephalopathy have worse prognosis.

•The presence of Wernicke encephalopathy should prompt the assessment of alcoholic liver disease.
